Nursing-facility Medicaid in the District
DHCF describes its Institutional Care Program as coverage for people receiving an institutionalized level of care in a nursing facility or an intermediate-care facility for people with intellectual disabilities. The current long-term-care eligibility page uses the same financial framework for long-term care: the 2026 Special Income Standard is $2,982 per month, published resource limits are $4,000 for one person and $6,000 for a married couple, and a medically needy spend-down alternative is available for people with high medical expenses (DHCF Types of Long-Term Care Services; DHCF Long-Term Care overview).
Financial approval is only part of the nursing-facility case. DHCF’s nursing-facility quick reference says a District resident must meet nursing-facility level-of-care criteria, typically through a comprehensive health assessment, as well as the financial rules. It also says individuals with serious mental illness or intellectual disability must have Preadmission Screening and Resident Review to determine whether nursing-facility admission is needed and appropriate (DHCF Nursing Facility Quick Reference).
