Who is and is not in the CHC pathway

DHS identifies nursing-facility residents paid by Medicaid and people receiving qualifying Attendant Care, Independence, Aging, or clinically eligible OBRA waiver services as CHC participants. PA Health & Wellness, who CHC serves State-operated nursing-facility residents, including State Veterans Home residents, and people in the ODP system are not eligible for CHC; LIFE is Pennsylvania's separate PACE alternative for qualifying adults 55 and older. Financial eligibility is a separate question

CHC delivery does not replace MA financial rules. DHS publishes a $2,982 monthly individual income figure for the 300%-FBR institutional pathway and also describes a medically needy spend-down option for applicants with income above that figure. PA DHS, Medicaid Payment for Long-Term Care

For an individual at or below that income figure, DHS describes a $2,000 resource limit plus a $6,000 disregard; a different $2,400 resource limit applies when monthly income exceeds $2,982. Choose the care setting before assuming the route

CHC can deliver nursing-facility care and home- and community-based services, but the care setting, clinical assessment, financial eligibility category, and care plan still matter. PA DHS, Apply for Community HealthChoices
