HOPE is the principal older-adult home-care waiver
The Home and Community-Based Options and Person-Centered Excellence, or HOPE, waiver provides home- and community-based services for South Dakotans at risk of institutionalization. It is a joint program of DHS Long Term Services and Supports and DSS South Dakota DSS eligibility page; HOPE waiver manual.
HOPE is aimed at people age 65 or older and adults with a qualifying disability who need nursing-facility level of care but can be served in a community setting. The waiver process uses the Home Care Assessment, an LTSS referral and assessment, a Medical Review Team level-of-care review, DSS financial eligibility, and an individual support plan/authorization process HOPE waiver manual.
