The CCC Plus Waiver is Virginia's main aging and physical-disability HCBS route
Virginia's Commonwealth Coordinated Care Plus Waiver, usually called CCC Plus, is a Medicaid home- and community-based services waiver. DMAS says it serves older adults, people with a physical disability, and people with chronic illness or severe impairment who would otherwise require nursing-facility admission, a prolonged hospital stay, or specialized-care nursing-facility care. Medicaid eligibility and LTSS screening are both required before waiver services are paid (DMAS CCC Plus Waiver).
A Community Based Screening Team of a social worker and Health Department nurse assesses CCC Plus need when the person is in the community. If the person is hospitalized, a discharge planner can conduct the screening. After Medicaid eligibility, screening, and waiver approval, the member can select waiver services and may choose agency-directed care, consumer-directed care, or both for services where those models apply (DMAS CCC Plus Waiver).
