Long-Term Care Funding in Alaska

Why Alaska long-term-care planning is different
Alaska Medicaid is also called DenaliCare in a current long-term-care eligibility reference. The Division of Public Assistance, or DPA, makes the financial determination and calculates a recipient's Cost of Care Amount, while Senior and Disabilities Services, or SDS, handles the functional assessment and level-of-care work for the relevant long-term-care pathway (2026 Alaska long-term-care eligibility guide; Alaska DPA COCA guide; Alaska SDS Assessment Unit).
Alaska is an income-cap state for Nursing Home Medicaid and HCBS waivers. A 2025 Alaska elder-law presentation describes a $2,901 gross-income limit that adjusts each January and permits a Medicaid Qualifying Income Trust, commonly called a Miller Trust; a 2026 eligibility guide reports the updated $2,982 monthly limit. Alaska's current figures and unusual transfer rule
The home needs two different analyses
Alaska has adopted the Uniform Real Property Transfer on Death Act. A properly recorded TOD deed is revocable, is nontestamentary, and does not create a beneficiary's legal or equitable interest during the owner's life; the statute also says it does not affect public-assistance eligibility during that life (Alaska Legislature, Uniform Real Property Transfer on Death Act).
That lifetime title rule does not eliminate the separate question of post-death Medicaid recovery. Alaska's recovery statute authorizes claims against an estate for specified institution and HCBS services received at age 55 or older, protects specified spouses and children, and the estate-recovery regulations give beneficiaries a written undue-hardship route (Alaska Stat. 47.07.055; 7 AAC 160.230).
Care at home, veterans, and insurance
Alaska operates five HCBS waivers, including the Alaskans Living Independently, or ALI, waiver for adults age 65 or older and adults with physical disabilities who meet nursing-facility level of care.
