Financial approval and service need are separate gates

For Alaska HCBS waivers, the Department of Health says applicants must meet Medicaid income and resource limits through DPA and must also be assessed for the applicable level of care. Nurses or other trained staff conduct the functional assessment by in-person interview or video call, and a nurse supervisor reviews the result before the care-level decision is made (Alaska HCBS waiver overview).

The SDS Assessment Unit identifies the Alaskans Living Independently, Adults with Physical and Developmental Disabilities, Community First Choice, and Personal Care Services pathways among its assessment responsibilities. It explains that a person meeting nursing-facility level of care may choose home and community-based services or institutional care, subject to program eligibility and service planning (Alaska SDS Assessment Unit).

A 2025 Alaska LTC presentation describes the practical sequence as DPA financial review followed by SDS medical and level-of-care work, with a care coordinator preparing a support plan. Income can lead to a Cost of Care Amount

Alaska does not simply ask whether a person has income. Its COCA guide says DPA adds monthly income, subtracts allowed expenses such as the personal-needs allowance and unreimbursed medical costs, and treats the remainder as the monthly Cost of Care Amount. For nursing-home and waiver applicants whose income is above the applicable income cap, Alaska recognizes a Qualifying Income Trust, also called a Miller Trust.
