Long-term care is both financial and clinical

Hawaii DHS states that a beneficiary seeking long-term-care services must meet both financial and clinical requirements. The clinical requirement is a minimum functional impairment described as institutional level of care, and the state says services may be furnished in a facility, in the member’s home, or in a community setting (Hawaii DHS long-term-care notice).

That structure matters because eligibility is not established by a diagnosis alone, and financial eligibility is not established by a functional assessment alone. The published 2026 Med-QUEST chart separates MAGI-excepted income standards, medically needy standards, and resource limits; the details can vary with household composition and coverage pathway (Med-QUEST 2026 income and asset standards).

People with intellectual or developmental disabilities may also encounter Hawaii’s separate Section 1915(c) Home- and Community-Based Services waiver. CMS lists that I/DD waiver as approved effective July 1, 2026 through June 30, 2031, distinct from the general QUEST Integration demonstration (CMS Hawaii I/DD HCBS waiver record).

Start with the correct agency and documents

For an application or renewal, preserve financial statements, proof of income, title documents, prior transfers, and current clinical records. A family should ask the relevant QUEST Integration health plan or Med-QUEST office which pathway, functional review, and service authorization applies before assuming that a general Medicaid approval also resolves long-term-care coverage.

Program names and managed-care structures can be confusing, but the key Hawaii point is straightforward: Med-QUEST administers Medicaid, QUEST Integration is the major delivery framework, and long-term-care eligibility combines financial rules with a clinical level-of-care determination.
