Louisiana has HCBS waiver pathways
Louisiana uses Section 1915(c) HCBS waivers to provide certain Medicaid-funded services outside an institution. CMS lists the Adult Day Health Care Waiver and Community Choices Waiver for people who meet nursing-facility level of care, in addition to Louisiana waivers for other populations and levels of care (CMS Louisiana waiver factsheet).
CMS says Community Choices serves people age 65 or older and people age 21 through 64 with physical disabilities who meet nursing-facility level of care.
