Louisiana Medicaid is the starting program

Louisiana calls its program Louisiana Medicaid, and the Louisiana Department of Health publishes the rules used to determine coverage. LDH's long-term-care information says the pathway can cover services in a Medicaid-certified facility or services at home in the community, provided the person satisfies the program's eligibility requirements (LDH long-term-care FAQ).

For institutional care, LDH's LTC policy identifies nursing facilities and intermediate care facilities for individuals with intellectual disabilities as institutional settings subject to the special-income-limit framework. An applicant also has to meet the nonfinancial and resource rules; a label such as “nursing home Medicaid” is not itself an eligibility determination (LDH H-800 LTC policy).

For community services, Louisiana uses Section 1915(c) waivers. CMS lists the Adult Day Health Care Waiver and Community Choices Waiver for older adults and people with physical disabilities who meet a nursing-facility level of care, while other waivers serve people with intellectual or developmental disabilities or other populations (CMS Louisiana waiver factsheet).

Income is not one simple rule

LDH's January 2026 chart sets the special income limit, or SIL, at $2,982 per month for one person and $5,964 for a couple. The policy describes comparing gross earned and unearned income before deductions to the SIL for institutional coverage, so the applicable program and household facts matter (LDH Z-700, January 2026; LDH H-800 LTC policy).

LDH's consumer information adds an important qualification: a person whose countable income exceeds three times the monthly SSI federal benefit rate may still qualify for long-term-care services if medical expenses are sufficient to spend down the monthly income. That published alternative is why Louisiana should not be summarized as a state in which every over-cap applicant necessarily needs the same Qualified Income Trust strategy (LDH long-term-care FAQ).

Resources and care need separate review

LDH's 2026 chart gives a $2,000 resource limit for an individual and $3,000 for a couple. The agency also explains that countable resources can include cash and other property convertible to cash, while some property may not count; ownership, availability, exemptions, and a spouse's status must be checked under the relevant policy rather than inferred from account title alone (LDH Z-700, January 2026; LDH long-term-care FAQ).

A functional or medical review is also separate from the financial work. Louisiana uses the Level of Care Eligibility Tool, or LOCET, and a Level I PASRR screen in the medical-eligibility application process for a Medicaid-certified nursing facility (LDH nursing-facility admission guide).

Practical point: Start by identifying the care setting and the precise Louisiana pathway. Then apply financial eligibility, transfer, level-of-care, and application rules to that pathway using current LDH materials.
