Michigan uses parallel long-term-care pathways

Michigan Medicaid is administered by MDHHS, but long-term services and supports arrive through several channels rather than one single long-term-care program. Nursing-facility Medicaid is the institutional route; MI Choice is the home- and community-based waiver route; and PACE integrates Medicare and Medicaid financing for people served by a PACE organization (MDHHS, MI Choice Waiver Program; MDHHS, PACE).

MI Choice is a federal 1915(b)/(c) program operated by MDHHS through enrolled waiver agencies that function as Prepaid Ambulatory Health Plans. Those regional agencies are part of the practical intake, assessment, and care-coordination structure (MDHHS MI Choice Provider Manual; MDHHS MI Choice provider page).

Eligibility has both financial and care-need components

For nursing home Medicaid, MI Choice, PACE, and MI Coordinated Health, Michigan uses a special income level of 300% of the SSI federal benefit rate: $2,982 per month in 2026. MDHHS's HCBS comparison chart expressly applies that figure to MI Choice, PACE, and MI Health Link/Coordinated Health enrollment (MDHHS HCBS Program Comparison Chart; Medicaid.gov 2026 Spousal Impoverishment Standards).

Michigan's eligibility rules are published in the Bridges Eligibility Manual, while MI Choice itself serves older adults and adults with disabilities who meet Michigan nursing-facility level-of-care criteria. Financial eligibility alone does not establish that level of care (MDHHS Bridges Eligibility Manual contents; MDHHS MI Choice Waiver Program).

Michigan's baseline is not Florida's income-cap model. MDHHS describes a Group 2 pathway in which incurred medical expenses can determine eligibility for people above a limit, while its LTC programs use the stated 300%-of-SSI special income level. Do not assume a Florida-style Qualified Income Trust answer applies in Michigan (MDHHS BEM 105; Florida Medicaid long-term care page).

MI Choice and PACE solve different access problems

PACE is a separate integrated model authorized under the federal Balanced Budget Act of 1997. The PACE organization enters a three-party agreement with CMS and MDHHS, so it is not simply a different name for MI Choice (MDHHS PACE).
