Aid and Attendance is a federal VA pension addition

The U.S. Department of Veterans Affairs describes Aid and Attendance as a monthly payment added to a qualifying VA pension for eligible Veterans and survivors. VA says a person may qualify if they need another person to assist with activities such as bathing, feeding, or dressing; must remain in bed much of the day because of illness; lives in a nursing home because of disability-related incapacity; or meets specified visual-impairment standards. It is a federal benefit with its own VA eligibility and application rules, not North Dakota Medicaid approval (VA Aid and Attendance).

VA also explains that Aid and Attendance and Housebound benefits cannot be received at the same time. A VA nursing-home claim may require VA Form 21-0779, while VA directs claims based on the need for regular aid and attendance to the appropriate pension application process. Veterans and survivors should use a VA-accredited representative or VA source for current evidence requirements and should separately disclose the income to HHS for Medicaid treatment (VA Aid and Attendance; VA Form 21-0779).

North Dakota has one state Veterans Home

The North Dakota Veterans Home is located in Lisbon. Its official site says it has served honorably discharged veterans and their spouses since 1893 and currently has 52 skilled nursing beds and 98 basic-care beds. This is one North Dakota state Veterans Home facility, not an automatic Medicaid or VA pension entitlement. Families should ask the Home about its admission, payment, care-level, and benefits-coordination rules for the individual resident (North Dakota Veterans Home).

A state property-tax credit may also affect the household budget

North Dakota's Disabled Veteran's Property Tax Credit is available to qualifying veterans with a 50 percent or greater service-connected disability, or qualifying total-disability compensation status, who have an ownership interest and reside in the property. The published table permits taxable-value reductions from $4,500 at 50 percent disability to $9,000 at 100 percent disability; a surviving spouse may qualify under the stated conditions. This is a tax credit, not a Medicaid resource exemption or a VA Aid and Attendance payment (North Dakota Tax Commissioner veterans credit).

VA pension, the state Veterans Home, the tax credit, and Medicaid are separate systems. HHS says most earned and unearned income is counted for elderly or disabled Medicaid, subject to listed exceptions and the individual calculation. Do not assume a VA payment is automatically excluded or that a tax credit changes the Medicaid client-share result; request a written HHS treatment of the actual benefit (North Dakota LTC Unit brochure; VA Aid and Attendance).
