Nursing-facility coverage runs through ALTCS

Arizona’s nursing-home Medicaid pathway is ALTCS. AHCCCS says ALTCS serves people who require nursing-facility level of care and may provide services in an institution; an eligible member does not have to be in a nursing home because ALTCS also serves qualified people in home and community settings (AHCCCS ALTCS overview). For an applicant who needs nursing-facility coverage, ALTCS medical eligibility and the financial application must both be resolved.

AHCCCS’s pre-admission screening policy is the key medical review. It says a person qualifies for ALTCS institutional or home and community-based services only with a nonpsychiatric medical condition or developmental disability that necessitates nursing-facility or intermediate-care-facility level of care, as determined through the PAS assessment (AHCCCS PAS general provisions). The PAS assessor is a registered nurse or social worker, and the score is compared with the relevant threshold for elderly/physically disabled or developmentally disabled applicants (AHCCCS PAS general provisions).

Share of cost and the personal-needs allowance

ALTCS uses a share-of-cost calculation for some members. AHCCCS says the personal-needs allowance, or PNA, is subtracted from counted income and is determined month by month according to the person’s living arrangement (AHCCCS ALTCS share-of-cost deductions). The policy’s 2026 standards table lists 15% of the Federal Benefit Rate as $149.10 and 300% of the Federal Benefit Rate as $2,982 (AHCCCS ALTCS share-of-cost deductions).

Rather than treating $149.10 as a universal payment rule, use it as the published 15%-of-FBR 2026 standard and have AHCCCS apply the actual living arrangement, month, support-garnishment facts, and permitted deductions. The policy expressly says the PNA depends on living arrangement and can be increased by garnished court-ordered child or spousal support, including administrative fees (AHCCCS ALTCS share-of-cost deductions).

Financial eligibility still comes first

AHCCCS’s 2026 chart lists an individual ALTCS income cap of $2,982 monthly and an individual resource limit of $2,000. It also identifies nursing-home level of care or equivalent and a possible share of cost as ALTCS requirements (AHCCCS February 2026 eligibility chart). Income above the applicable gross-income test may be addressed through an Income-Only or Miller trust when the trust conditions apply; the trust cannot be funded with resources (AHCCCS Income-Only Trust policy).

A transfer penalty can leave the person eligible only for a limited ALTCS service package while long-term-care services are not covered. AHCCCS calculates that penalty by the applicable geographic private-pay rate and says there is no limit on the penalty period’s length (AHCCCS transfer-penalty policy).

Institutional-care checklist: PAS level-of-care decision, current income and resources, trust review if income exceeds the cap, share-of-cost budgeting, and a five-year transfer review. The current published 15%-of-FBR 2026 PNA standard is $149.10, but the actual monthly PNA depends on the living arrangement (AHCCCS share-of-cost policy).
