Care assessment and financial pathway
HCBS applicants must meet nursing-facility Level of Care. Missouri's September 2024 manual says DSDS staff or a designee complete an assessment using the InterRAI HC tool and that a score of 18 or more determines the person qualified for that level of care (Missouri Level-of-Care Manual).
In the financial rules, Missouri's July 2026 Eligibility Standards lists a $1,737 monthly HCB income standard for an eligible person and a $6,220.50 resource maximum after Division of Assets. Missouri's HCBS manual says an irrevocable Qualified Income Trust can reduce countable income for a person who is otherwise eligible for a QIT pathway; that is not a reason to assume every higher-income applicant should use one without FSD confirmation (Missouri Eligibility Standards, July 2026; Missouri HCBS Manual, Appendix 1).
Waitlists and enrollment caps matter
The Independent Living Waiver is a self-directed option for adults with disabilities. Missouri says each waiver year has a limited number of slots; when they are filled, otherwise eligible people are placed on a waitlist prioritized by level-of-care scores, and openings are used at the beginning of a new waiver year (Missouri HCBS Manual).
The Structured Family Caregiving Waiver is available for people with Alzheimer's disease or related dementia disorders and is limited to a specified number of unduplicated participants per waiver year. Missouri's manual says that once a slot is filled, it cannot be reused in that waiver year even if the participant leaves; the manual does not state a public numerical cap in the cited section (Missouri HCBS Manual).
Choosing a route
Home care can preserve a person's setting and routines, but it is not automatically available merely because nursing-home placement is unwanted. Assessment, age/program rules, financial eligibility, authorization, capacity, provider availability, and the one-waiver-at-a-time limit can all affect the result.
