Income: spend down for Medicaid, QIT for an HCB pathway

Missouri is not accurately summarized as a single hard-cap nursing-home state. Its July 2026 non-MAGI chart says aged and disabled Spend Down has no income limit but requires income to be spent down below the non-spend-down amount; for Vendor Care, available income is paid as surplus after the $50 allowance and allowable deductions (MO HealthNet Non-MAGI Eligibility Chart, July 2026).

Home and Community Based Medicaid has a separate $1,737 monthly income standard for an eligible individual in 2026. Missouri's HCBS manual says a Qualified Income Trust, also called a Miller Trust, can reduce countable income for someone who otherwise meets a QIT program's requirements, and the trust must be irrevocable, composed solely of the individual's income, and contain state-payback language (Missouri Eligibility Standards, July 2026; Missouri HCBS Manual, Appendix 1).

Home equity and the community spouse

For Vendor, HCB, or PACE eligibility, Missouri's July 2026 chart lists a maximum home equity interest of $752,000. That figure concerns eligibility; it does not decide later liens, estate recovery, title, or transfer consequences (Missouri Eligibility Standards, July 2026).

When one spouse needs nursing-facility or HCB services, Missouri calls its spousal resource process a Division of Assets. The 2026 state chart lists a minimum spousal share of $32,532 and a maximum of $162,660; it also lists a minimum monthly maintenance needs allowance of $2,705 and maximum of $4,066.50. Missouri says a spouse may seek a hearing or appeal regarding the spousal-share determination (Missouri Eligibility Standards, July 2026; Missouri Prevention of Spousal Impoverishment).
