MO HealthNet is Missouri's Medicaid framework

Missouri calls its Medicaid program MO HealthNet. For the aged, blind, and disabled population, the Department of Social Services' Family Support Division determines Medicaid eligibility; the state HCBS manual separately describes the MO HealthNet Division and the Department of Health and Senior Services as partners in long-term-care services (Missouri HCBS Manual, Appendix 1; Missouri Aged and Disabled Waiver).

For a Medicaid-certified nursing facility, Missouri calls the payment arrangement Vendor Care. The state eligibility chart describes Vendor Care as care in a nursing facility, an institution for people with intellectual disabilities, or certain mental or psychiatric hospitals; it describes the payment as MO HealthNet-covered services plus payment to the facility beyond the participant's responsibility (MO HealthNet Non-MAGI Eligibility Chart, July 2026).

Financial eligibility does not use one simple rule

Missouri's 2026 materials distinguish non-spend-down Medicaid, spend-down Medicaid, Vendor Care, and Home and Community Based services. The July 2026 chart lists a $1,131 monthly non-spend-down income limit for one aged or disabled person and says Spend Down has no income limit if income is spent down below that standard; the same chart identifies a $6,220.50 individual resource maximum effective July 1, 2026 (Missouri Eligibility Standards, July 2026).

Vendor Care works differently from a conventional income-cap description. Missouri's chart says all income is paid to the facility as surplus after the $50 personal-needs allowance and permitted deductions, while the consumer Spend Down page explains that someone age 65 or older, disabled, or blind may obtain MO HealthNet for a month by meeting a monthly spend-down amount (Missouri Eligibility Standards, July 2026; Missouri Spend Down).

Care need is a separate decision

Financial eligibility alone does not establish HCBS or nursing-facility coverage. Missouri's HCBS manual says an applicant for HCBS must meet nursing-facility Level of Care, determined during a DSDS assessment or reassessment using the InterRAI HC tool; a score of 18 points or more qualifies for that level of care in the manual's process (Missouri Level-of-Care Manual).

For a person age 63 or older who can remain home, the Aged and Disabled Waiver offers homemaker, chore, respite, home-delivered meals, and adult-day-care services when assessed needs would otherwise require nursing-home care (Missouri Aged and Disabled Waiver). The correct first question is therefore which care setting and eligibility pathway apply, rather than whether a generic “Medicaid” rule fits every Missouri family.

Key distinction: Missouri's Vendor Care uses a patient-liability/surplus calculation, while HCB Medicaid has its own $1,737 monthly income standard in 2026. Confirm the particular pathway with FSD before moving assets or income (Missouri Eligibility Standards, July 2026).
